No shirt, no service

I have noticed a disturbing trend of clinicians and clinic staff taking BP measurements and listening to heart and chest sounds through clothing. Had I done that in my pulmonary practice, I never would have been able to identify rales, rhonchi, or S1, S2, S3 heart sounds.
