This patient was discovered to have gallstone ileus, a rare complication of cholelithiasis that has a high rate of mortality. An unusual process must occur in order to cause this complication.
Gallstones of this size (typically >2 cm) can enter the bowel following the development of a fistula between the gallbladder and the small bowel. These fistulas can be asymptomatic, and the first signs of illness are usually related to intestinal obstruction.
