A 54-year-old man complained of fevers and abdominal pain. Four days earlier, he had noted intense left upper-quadrant abdominal pain that was worse with inspiration and a throbbing pain in the distal left fifth digit. Two days later, an extremely tender, purple nodule appeared on the same finger. A 0.8-cm red-to-violaceous papule was visible on the medial aspect of the pulp of the distal left fifth digit. The papule had an erythematous base, was tender to palpation, and was slightly indurated.
